There are many ways to register (Pre-Registration):

  1. Visit the dental practice in person.
  2. Contact the practice on 020 8749 7427 and book a new patient registration appointment
  3. E-mail the practice on goldhawkdentalpractice@outlook.com  with your full details:
  • Full Name
  • Date of Birth
  • Email
  • Phone number
  • Address Details
  • Any family members to register
  1. Complete the patient registration form.
